Model Release Form

1. Grant of rights. In consideration of my engagement as a model, and for other good and valuable consideration, the receipt of which I acknowledge, I grant Dog Haus, together with its affiliates, franchisees, legal representatives, successors and assigns, and anyone acting with its authority or permission (together, the "Released Parties"), the irrevocable, perpetual, worldwide right to photograph, film and record me, and to copyright, use, reuse, publish, republish, broadcast and rebroadcast the resulting photographs, video and audio recordings of me or in which I appear (the "Images"). The Images may be used in whole or in part, alone or in composite form, and may be edited, altered or distorted in character or form from time to time without restriction. The Images may be used with my own name or a fictitious name, in any media now known or later developed, for advertising, trade, promotion or any other lawful purpose.

2. Related materials. I consent to the use of any advertising copy, voiceover, broadcast or printed matter in connection with the Images.

3. Waiver of approval. I waive any right to inspect or approve the Images, any finished product, or any copy, voiceover or printed matter used with them, and any right to approve how they are used.

4. Release. I release, discharge and agree to hold harmless the Released Parties from any liability arising from the Images, including any blurring, distortion, optical illusion or use in composite form, whether intentional or not, that may occur in taking the Images, in any later processing, or in any broadcast or publication of them. This release applies even if the use subjects me to minor ridicule, scandal or indignity.

5. Ownership and compensation. The Images are the sole property of Dog Haus. I will receive no compensation for their use beyond what is stated in my engagement with Dog Haus.

6. Age and authority. I warrant that I am at least 18 years old and have full right and authority to enter into this agreement in my own name.

7. Acknowledgment. I have read this release before signing it and fully understand its contents.
